Art director Hirofumi Nakaoka explained in an interview with Game Informer that the villain was created as a midpoint between the human character and the religious horror motif inspired by “Los Iluminados”.

According to Nakaoka, the character’s cow head is a reference to previous Resident Evil games. It’s where cow and deer heads were often used as decorations in various settings. Nakaoka was quick to point out that it’s a mask and the character is not a Minotaur, so players shouldn’t expect to see the cow’s eyes moving. In fact, it looks a bit cumbersome for the man wearing the mask! Nakaoka leaves it open whether the man put the dead cow’s head on himself or if it was put on by someone else. Regardless, it’s pretty creepy and looks like an intriguing new addition to the game’s roster.
